Leiomyomas are the most common benign gynecological tumor with only 1-2% confined to the cervix. The paucity of smooth muscles in the cervical stroma makes leiomyomas in the cervical area uncommon. We report a case of large, 20 weeks size central cervical fibroid in a 43 years old seropositive parous women who underwent abdominal hysterectomy after carefully delineating the anatomy of pelvis. Intraoperatively central cervical fibroid with a typical “lantern on top of St. Paul’s cathedral” appearance was seen which was deeply impacted in pelvis, making it difficult for removal using routine techniques like finger dissection andmyoma screw, so blades of outlet forceps were used to perform a successful myomectomy which was followed by hysterectomy without any complications.
