ABSTRACT Postoperative case following a vaginal procedure, presenting with bubbles in the urine, can be a real nightmare for the operative team. Emphysematous cystitis is a rare form of complicated urinary tract infection (UTI) characterized by the presence of gas within the bladder as well as inside the bladder wall. It is life-threatening if not identified and treated in time. The aim of reporting this case is to orient the fellow practitioners with this rare presentation—it’s cause, diagnosis, management and prevention. To the best of our knowledge this is the first case of emphysematous cystitis to be reported, following a vaginal surgery with differential of trauma secondary to catheterization and fistula.
