Acute promyelocytic leukemia (APL) during pregnancy is a rare comorbidity and this can lead to adverse outcomes without timely diagnosis and appropriate management. Timely diagnosis and medical management for APL during pregnancy remains challenging. We report a patient with no regular prenatal visits who was diagnosed with APL during the workup of severe anemia in her third trimester of pregnancy. Although supportive measures and chemotherapy were administered soon after terminating the pregnancy, the management was complicated with differentiation syndrome, DIC and sepsis. The outcomes of APL in pregnancy depend greatly on the timely diagnosis and appropriate management of the disease.
