The Effectiveness of a Simulation based Structured Stepwise Approach to Diagnostic Laparoscopy in Gynecology Trainees: A Prospective Study

Abstract

Background: Simulation training is well integrated into laparoscopic surgical training. However, data on an objective evaluation of a structured and stepwise approach to simulation training remain scarce.

Aim: To investigate the effectiveness of a proposed simulation-based structured stepwise diagnostic laparoscopy training module (SSTM) for novice gynecology trainees.

Materials and methods: A prospective study was conducted among gynecology trainees (level 1 to 3) at a University Hospital in London/ North East and North Central London deanery. Participants underwent a pre-test evaluation followed by the SSTM and post-test assessments. The pre- and post-test assessments were identical and consisted of a written component for knowledge, a self-assessment form for reaction evaluation and a skill assessment (containing an objective and a subjective component). The pre-test, intervention and post-test I were performed on the same day, with another post-test (post-test II) conducted 6-8 weeks after the SSTM to assess for retention of knowledge and skills. The impact of the SSTM and retention of knowledge and skills were measured by analyzing score differences between pre- and post-test I, and between post-test I and II, respectively.

Results: The SSTM significantly improved the level of knowledge, skills and self-evaluation in study participants (p=<0.001 for all aspects). The skill was retained at 6-8 weeks post-SSTM with no significant decline for either objective (p =.876) or subjective (p =.071) assessment.

Conclusion: This study concluded that the SSTM is an effective tool for laparoscopic simulation in novice gynecology trainees
